Free AI audit for your clinic.

No product pitch. A no-nonsense method to identify the workflows costing you the most, calculate the real ROI of automation, and decide whether Orkelia (or a competitor) is worth it for your case.

Step 1

Count what comes in.

Open your professional Instagram. Go to the DM inbox. Count how many messages you received last week outside opening hours (before 9am, after 7pm, Saturday evening, Sunday).

Do the same for WhatsApp Business if you have it. And for your website's contact form (look at the timestamps).

  • How many Instagram messages outside hours over 7 days?
  • How many WhatsApp messages outside hours over 7 days?
  • How many form submissions outside hours over 7 days?
  • How many of these messages contain a booking intent or a commercial need?
Field insight: across 30 aesthetic clinics observed, the average runs at 12-18 out-of-hours requests per week, of which 60-70% with a clear commercial intent (booking request, pricing question, info need).
Step 2

Measure the response time.

For the last 10 messages from potential patients, calculate the time elapsed between their message and your reply. Be honest.

Now the question that stings: how many of these patients had already booked elsewhere by the time you replied? If you contacted them again today to ask, most would answer "I've already found someone".

Sector benchmark: in aesthetic medicine, 47% of patients book with the first clinic that replies. If you reply in 3h vs 24h, you capture 3 to 5× more bookings from the same volume of requests.
Step 3

Calculate the net loss.

Based on your steps 1 and 2, here is a simple calculation:

Out-of-hours requests / week15
Booking intent (60%)9
Patients lost (50% book elsewhere)4-5 / wk
Average spend consultation + procedure€300
Estimated monthly loss€5,400
Annual loss€64,800

At this point, the arithmetic speaks for itself. An automation at €497-697/month costs less than a single week of lost patients.

Step 4

Identify the 3 priority workflows.

You can't automate everything at once, and no one should. Prioritize:

  • Out-of-hours capture — the No. 1 topic everywhere. Insta DM + WhatsApp + website form.
  • Day-before confirmation, anti no-show — direct revenue theft, typical no-show rate 12-18% in aesthetics.
  • Reactivation of dormant patients >3 months — the highest ROI: your existing list.

Only tackle the 4th workflow (post-treatment follow-ups, satisfaction tracking, etc.) once the first 3 are stable.

Step 5

Define the escalation rules.

An AI agent must NEVER touch certain conversations. List them before signing with anyone:

  • Life-threatening urgency keywords (severe pain, hemorrhage, severe allergy) → immediate escalation
  • Minors without a guardian → systematic escalation
  • Financial requests (refund, invoice dispute) → mandatory escalation
  • Complaints / reports → escalation + compliance log
  • Quality / GDPR claims → escalation to the DPO
Critical safeguard: require the provider to make these rules configurable BEFORE activation, not after an incident. Ask to see the urgency classification system in the demo.
Step 6

Choose the right tool.

Three broad categories on the market in 2026:

1. Booking-only solutions (Doctolib, Maiia, Calendly Pro) — respond ONLY to explicit booking requests on their own platform. They don't read Instagram, WhatsApp, or your website.

2. Generic chatbots (Tidio, Crisp, Intercom + custom GPT) — respond to everything, but with a recycled GPT lacking domain knowledge. You pay €30-150/month but have to configure everything yourself.

3. Verticalized AI agents (Orkelia, a few rare competitors) — they know your field, handle escalations, run as a managed service. Pricing of €497+/month but zero effort on your side after setup.

Choose the category based on your available time: if you have 2-3h/week to configure a chatbot, go for category 2. If you want it to work without having to think about it, category 3.

Step 7

Test before you pay.

Before signing a 6-month or 12-month commitment, require:

  • Free trial without a card — at least 14 days to calibrate the tone.
  • Written money-back guarantee — if the priority workflow isn't operational by day 30, full refund. No justification needed.
  • Review mode you can enable — you validate every message before sending during the trust phase.
  • 1-click deactivation — no 3-month cancellation procedure.
  • Full export of your data — JSON + CSV if you leave.

Any provider that does NOT offer these 5 elements doesn't believe in its own product. Move on.
